The Uribe Regional Reference Centre (CCR) is organising the course “Personalised Care in Residential Care Homes”, a training programme comprising four sessions for care homes for older people in the Uribe area.

These sessions will cover the tools and methods that will facilitate the implementation of the personalised care model. In addition, during the training sessions, the needs of care homes and existing examples of good practice will be gathered. The sessions will also address coordination between health and social care services, which is essential to ensuring continuity of care for residents.

Theoretical and practical training for care home professionals

During the first session, to be held on 27 October at the NIC headquarters in Urduliz, the training programme on personalised care in residential care homes in the area will be outlined. During this session, the results of the self-assessments carried out by participating care homes since June will be presented. The proposed programme of work for the remaining sessions will also be presented.

During the second session, scheduled for 3 November, participants will explore various tools used in the new personalised care model. This session will combine theoretical content with group activities followed by a plenary discussion. The aim is to give care home professionals an opportunity to share experiences and examples of good practice from their day-to-day work.

During the third session, scheduled for 10 November, participants will explore the Personalised Care and Life Plan in care homes for older people. Theoretical content will be combined with group activities followed by a group discussion.

During the final session, to be held on 17 November, participants will explore both Advance Planning for Decisions and Care and Health and Social Care Coordination.

Care homes must register in advance to attend these sessions.
