This is a collaborative working space aimed at analysing, reviewing, and proposing improvements to shared care pathways and protocols for people living in residential care centres. The session is designed to promote joint reflection and the exchange of experiences among the different stakeholders involved, with the aim of strengthening coordination, optimising care processes, and addressing current needs identified in the residential care setting.

Organised jointly by the CCR and the Uribe OSI, this meeting continues the ongoing work in socio healthcare coordination, identifying shared care pathways and improving the barriers detected in their implementation.

The meeting is intended for participation by residential care professionals with healthcare profile.
